Patient Enrollment

To enroll in Premier Medical Services, patients must reside in a residential community or program that partners with Premier Medical Services. Discuss your eligibility with your community staff.

Complete Enrollment Forms:

Electronic Enrollment:

Visit the Premier Medical Services website and complete the electronic enrollment form. Once you’ve completed the electronic forms, submit them through the secure link provided on our website.

Paper Enrollment:

If you prefer to complete your forms on paper, download the Patient Enrollment Form, complete and sign where required. Fax signed paperwork to us at 817-753-1090 or give a copy to the nurse at the community, or scan and upload them securely through our website’s document submission portal.

Confirmation of Enrollment:

Once enrollment is confirmed, Premier Medical Services will begin coordinating your care with your residential community, and you will be contacted to schedule your first visit.
